Vital Registration

For assistance with birth, death, and marriage certificates, and corrections to these vital certificates, call (304) 558-2931 or email dhhrvitalreg@wv.gov.  Documents may be faxed to (304) 558-1051.

The office has a walk-up counter located on the right side of the lobby on the first floor of 350 Capitol Street in downtown Charleston (corner of Capitol and Washington streets). There is metered street parking on Capitol Street. The office is open Monday through Friday from 8:30AM - 5:00PM, except State holidays. Please note wait times may be higher during the lunch hour.
