Hopemont Hospital accepts referrals for nursing care patients from hospitals, physicians, other nursing facilities and individuals.
Admission time and date are flexible at convenience of family.
Application Procedure:
Under Federal OBRA regulations, all applicants for admission to any nursing facility accepting Federal funds must undergo Pre-Admission Screening. Any hospital, most physicians, and any DHHR office, can provide the PAS-200 form required.
