The Bureau for Medical Services has four Waiver Programs:
The Aged and Disabled Waiver (ADW) is for people age 18 or older who meet the medical criteria of receiving nursing home care and who meet financial requirements.
The Intellectual/Developmental Disabilities Waiver (IDDW) is for individuals who have intellectual disabilities and/or developmental disabilities who would otherwise be institutionalized.
The Children with Serious Emotional Disorder Waiver (CSEDW) provides support to children with severe serious emotional disturbances by helping to keep them with their families, in the home and with a support network while receiving the services they need to improve their outcomes.
The Conflict-Free Case Management (CFCM) Program will allow ADW, IDDW and TBIW participants to receive case management from one agency and all direct services from a different agency.
The Substance Use Disorder (SUD) Waiver addresses the immediate and long-term physical, mental and social needs of individuals with substance use disorders, and promotes and sustains their long-term recovery.
The Traumatic Brain Injury Waiver (TBIW) is for people age 3 or older who have a traumatic brain injury defined as a non-degenerative, non-congenital insult to the brain caused by an external physical force resulting in total or partial functional disability and/or psychosocial impairment or an injury caused by anoxia due to near drowning and who meets the medical criteria of receiving nursing home care.

Non-Emergency Medical Transportation
Non-Emergency Transportation (NEMT) is available to Medicaid members who need assistance in order to keep scheduled medical appointments and treatments. In order to be eligible for NEMT, a person must be a Medicaid member and have an appointment for medical treatment that is approved under Medicaid guidelines.
For more information, to request gas mileage reimbursement or schedule a trip, please call the Medicaid NEMT broker, Modivcare at 1-844-549-8353, Monday - Friday between 7:00am to 6:00pm. Call at least five business days before your appointment. You may also schedule a trip on their website, please click here for more information.
You will need to have the the member's name, Medicaid ID number, home address, phone number. You will also need your healthcare provider's name, phone number and address, and the the date and time of your appointment. Also, please let the Modivcare representative know if you have any special needs such as a wheelchair accessible vehicle, assistance during the trip or someone to ride with you.
