​​The Division of Health Promotion and Chronic Disease (HPCD) takes a comprehensive approach to chronic disease management and prevention, working with communities, health systems, and decision-makers​ to make it easier for people to be healthy in the places they live, work, and play.

​HPCD VISION

Creating collaborative impact to achieve equity, health, and wellness for all West Virginians.

HPCD MISSION 

The mission of HPCD is to create intentional change to improve health for all West Virginians by:
  • ​Facilitating a collaboration of diverse partners through community engagement to reduce the burden of chronic disease in WV;
  • Identifying and challenging social inequities by addressing all social determinants of health;
  • Increasing the capacity of communities to create sustainable and equitable policy, systems, and environmental changes to improve health and wellness across all stages of life.

HEALT​H EQUITY DEFINITION

Health Equity is defined as the absence of unfair and avoidable differences in health. Health equity requires removing obstructions to health, such as poverty, discrimination, and their consequences so no one is disadvantaged from achieving their full health potential because of social position or other systemic barriers.

​HEALTH EQUITY STATEMENT ​

HPCD is committed to:
  • Listening to and partnering with communities to address the issues they have identified as most important;
  • Removing barriers to being healthy by addressing structural inequality, health disparities and the Social Determinants of Health;
  • Ensuring all programming is culturally competent and informed;
  • Recognizing our own limitations and personal biases and continued evaluation and growth of our health equity knowledge and strategies.
