Chronic diseases are the most common and costly of all health problems—and they are also the most preventable.
HPCD focuses on the prevention and management of chronic diseases in West Virginia. We do this by uniting communities, decision-makers and health care systems to combat chronic diseases in the places we live, work, and play.
