​​​​​​​ The Division of Health Promotion and Chronic Disease (HPCD) takes a comprehensive approach to chronic disease management and prevention, working with communities, health systems, and decision-makers to make it easier for people to be healthy in the places they live, work, play and pray.

Our Goals

 
West Virginians will need to work together to make the greatest impact on improving health outcomes. Each of us can model and promote healthy behaviors and advocate for healthy environments where we live, work, play and pray. All organizations have a role in promoting healthy environments that change the context for health. Working comprehensively together will help enhance a culture of health.

The Division of Health Promotion and Chronic Disease (HPCD) takes a comprehensive approach to chronic disease management and prevention, working with communities, health systems, and decision-makers to make it easier for people to be healthy in the places they live, work, play and pray.

In recognition of World Heart Day, the staff at The West Virginia Bureau for Public Health, Division of Health Promotion and Chronic Disease dressed in red clothing for a team meeting. Each team member shared what heart healthy activity they were going to complete on World Heart Day 2021. Examples included: 
  • thirty minutes of aerobic activity 
  • eating five servings of fruits and vegetables
  • running a mile at the gym

"Created by the World Heart Federation, World Heart Day informs people around the globe that CVD, including heart disease and stroke, is the world’s leading cause of death claiming 18.6 million lives each year, and highlights the actions that individuals can take to prevent and control CVD. Greetings all West Virginians!

The West Virginia Diabetes Action Plan Task Force has completed it's recommended Actions To Improve Diabetes Outcomes in West Virginia. This brief contains five recommendations to reduce the prevalence of diabetes in West Virginia. The West Virginia Diabetes Action Plan 2020 seeks to address diabetes and associated chronic diseases through long-term strategies designed around the key aspects of prevention, management, and access to support education, skills and behaviors for care.

West Virginia Health Care Teams Call to Action
Referrals are needed for National Diabetes Prevention Program (NDPP) participants.

To help you encourage COVID-19 prevention and facilitate overall wellness, the National Association of Chronic Disease Directors has released a fact sheet "Chronic Disease and COVID-19: What You Need to Know", written for the general public that relies on the current CDC guidance as well as draws from other medical advisory groups.
