The Take Me Home Transition (TMH) Program's Virtual Presentation will take place on December 12, 2023 from 9:00am until 11:00am. The presentation will feature information on the Take Me Home, West Virginia and the extended Telehealth pilot in addition, registered nurses, social workers and licensed professional counselors will have the opportunity to earn two CEU credits.
