The West Virginia Bureau for Medical Services (BMS) Behavioral Health Health Home for eligible Medicaid members with Bipolar disorder and at risk of having or having Hepatitis B or C was expanded statewide on April 1, 2017. BMS is seeking providers who are interested in becoming a Health Home provider. Health Home provider standards and rates are listed below.
In order to become a Health Home provider you must submit a Health Home Provider Application and narrative, see form below . The narrative addresses the following:
Describe your practice and your practice relationships with other health care providers in your area and why your practice is suited to be a Health Home provider.
- Describe your ability to capture member information and coordinate care for the Health Home population.
- Describe your ability to report the quality measures as outlined in the State Plan Amendment.
