Effective June 15, 2016, WV Medicaid will place an age limit on all tramadol-containing agents. On the effective date, all WV Medicaid Pharmacy point-of-sale claims for members less than 18 years of age will require Prior Authorization (PA). Claims for non-preferred tramadol-containing products will continue to require a PA for WV Medicaid members of all ages. You may submit PA requests to the WV Medicaid Rational Drug Therapy Program at 1-800-847-3859.
