West Virginia Medicaid and Children’s Health Insurance Program (WVCHIP) are required to provide a copy of 1095-B Health Coverage tax documents to the Internal Revenue Service (IRS) and to individuals who have received Medicaid and/or WVCHIP healthcare coverage during the calendar year 2022.
To request a copy of your 1095-B Health Coverage Tax document, please contact your local West Virginia Department of Health and Human Resources (DHHR) office.
