The West Virginia Department of Health and Human Resources,
Bureau for Social Services, is now soliciting applications for new
program models in the Transition to Adulthood Program.
The
goal of the Transition to Adulthood Program is to
sustain and further develop qualified Transitional Living Specialists statewide
that will provide supportive services for youth that fit the eligibility criteria under the John F. Chafee Foster Care Program for
Successful Transition to Adulthood
Agencies wishing to submit applications must submit a Letter of Intent to Apply by June 9, 2023.
Failure to submit the Letter of Intent to Apply will remove the
application from consideration. Applications must be received by the
Bureau by 4:00pm on June 16, 2023. Applications received after that date will be removed from consideration.
Agencies

The annoucement provides as detailed description of the services the Department wishes to make available to qualified customers in the specified counties. The grant award will be for the period of July 1, 2023 through June 30, 2024. The Agreement(s) resulting from this award will be performance based.
